Mammography Machine Cost & Buying Guide (2026)

Mammography room with a Hologic digital mammography system at the Humacao, Puerto Rico imaging center served by Medical Outfitters

Updated 29 September 2026. Prices are Medical Outfitters ranges for a system delivered, installed and warrantied, quoted individually for every site. Specifications and regulatory references were checked against manufacturer documentation and primary sources. Written by the Medical Outfitters sales and field engineering team.

A mammography machine costs about $40,000 for a refurbished 2D digital system and up to $500,000 or more for a new 3D tomosynthesis system. A refurbished 2D system runs $40,000 to $95,000, a refurbished 3D system $95,000 to $225,000, a new 2D system $110,000 to $220,000 and a new 3D tomosynthesis system $250,000 to $500,000, delivered, installed and warrantied. A certified refurbished system saves 40 to 60 per cent against new.

A mammography machine, also called a mammogram machine, is a specialized low-dose X-ray system used to image breast tissue for cancer screening and diagnosis, available in 2D digital and 3D tomosynthesis configurations. This guide covers what a mammography machine costs by type and by model, what moves the price, what a quote includes, the other costs of running a breast imaging program, 2D versus 3D versus breast CT, the systems we carry and how to decide between new and refurbished. Medical Outfitters sells Fujifilm and Planmed mammography systems new as an authorized dealer, supplies the Koning Vera breast CT, sources refurbished Hologic, GE and Siemens systems on request, and services mammography systems bought anywhere.

Pricing

How Much Does a Mammography Machine Cost?

Mammography machine cost is driven mainly by 2D versus 3D capability and by condition. The figures below are Medical Outfitters ranges for a system delivered, installed and warrantied. Headline prices published elsewhere are often for the equipment alone, before delivery, rigging, installation and current tariffs, so they read lower than a real all-in cost. Every Medical Outfitters system is quoted individually.

Mammography typeRefurbishedNewWhat you get
2D digital (FFDM)$40,000 to $95,000$110,000 to $220,000Full-field digital screening and diagnostic mammography
3D tomosynthesis (DBT)$95,000 to $225,000$250,000 to $500,000Tomosynthesis with synthesized 2D, optional contrast and biopsy

Inside each band the detector generation, the tomosynthesis and contrast licences, the biopsy option and the workstation decide where a quote lands. The same question arrives as mammography machine cost, mammogram machine cost, mammography machine price, cost of a mammography unit and mammography equipment cost, and the table answers all of them. By Model

Mammography Machine Price by Model: Hologic, Siemens, GE, Fujifilm and Planmed

The models below are the ones buyers ask about most. Each range is a Medical Outfitters figure for the system delivered, installed and warrantied with a standard paddle set and acquisition workstation. Fujifilm and Planmed systems are sold new by Medical Outfitters as an authorized dealer. Hologic, GE and Siemens systems are not sold new by us: their refurbished ranges are for systems sourced on request and refurbished to the same delivered, installed and warrantied standard, and the new figures are estimates of current OEM pricing so you can compare like with like.

Two things to read alongside the table. First, the price of a refurbished 3D system is set by the tomosynthesis licence, the detector generation and whether the biopsy option is fitted, more than by the badge on the gantry. Second, a system that is no longer manufactured can still be an excellent purchase if the manufacturer supports the platform and parts are available; a system that is newer but out of support is the harder one to keep certified.

Price Drivers

What Moves the Price of a Mammography Machine?

Two systems of the same age and the same brand can be tens of thousands of dollars apart. These are the things that move a mammography machine price, roughly in order of impact.

  • 2D or 3D Tomosynthesis is the single largest step. It needs a tube and gantry that sweep through an arc, a detector fast enough to capture the projections and a reconstruction engine, and it is licensed separately on most platforms
  • Detector Direct-conversion amorphous selenium and indirect caesium iodide detectors, pixel size and detector age. The detector is the most expensive component and the one that ages, which is why detector generation drives refurbished pricing
  • Synthesized 2D, contrast and biopsy options Synthesized 2D from the tomosynthesis volume, contrast-enhanced mammography and an upright stereotactic or tomosynthesis-guided biopsy option are each licensed or fitted separately
  • Software and AI Computer-aided detection and AI triage or density software are licensed separately and rarely transfer with a used system
  • Workstation and monitors The acquisition workstation comes with the system; a radiologist reading workstation with 5 megapixel mammography monitors is usually a separate line
  • Paddles and positioning aids A full paddle set, curved compression paddles and magnification stands are quoted per item on used systems
  • Condition and platform support Tube exposure count, detector calibration history, software level and whether the manufacturer still supports the platform matter more than the year of manufacture
The Quote

What Is Included in a Mammography Machine Price?

Before you purchase a mammography machine, compare what is actually in the two quotes. The gap between a cheap one and an expensive one is usually scope rather than the machine.

LineWhat to confirm
The systemGantry, tube, detector, standard paddle set and the acquisition workstation, with the tomosynthesis, synthesized 2D, contrast and biopsy options listed by name
Licensed softwareWhich options are enabled on this serial number, and whether they transfer on a used system
Delivery, rigging and installationIncluded in every Medical Outfitters quote, with calibration to manufacturer specification
Physics and MQSAWho coordinates the medical physicist’s acceptance survey and the MQSA paperwork before the first patient
Applications trainingOn-site training for technologists, especially for tomosynthesis positioning and biopsy
WarrantyTerm, and whether the detector and the tube are covered
Quoted separatelyReading workstation and 5 megapixel monitors, PACS and CAD or AI software, room shielding where required, extra paddles, biopsy add-on, accreditation fees

We handle delivery and logistics and installation and de-installation with our own crews, including to Puerto Rico and the Caribbean, where we installed the first Planmed Clarity 3D mammography system in the Dominican Republic.

Other Costs

What Else Does a Mammography Program Cost?

The system is the largest line, but a mammography program has recurring costs that a CT or ultrasound room does not, because mammography is the one imaging modality with its own national quality law. Budget for these alongside the purchase.

  • MQSA certification Every facility performing mammography in the United States must be certified under the Mammography Quality Standards Act, which requires accreditation by an FDA-approved body, an annual inspection and personnel qualifications for technologists, radiologists and physicists
  • Accreditation ACR mammography accreditation or another FDA-approved body, with phantom and clinical image review at accreditation and reaccreditation
  • Medical physicist An acceptance survey when the system is installed and an annual survey after that, plus the technologist’s quality control tests on the schedule the manufacturer’s QC manual sets out
  • Reading workstation and monitors Diagnostic reading on 5 megapixel mammography monitors, which we supply alongside the system
  • Service contract Priced against the platform and its age; whether the detector and the tube are covered is the clause that decides whether a contract is complete
  • Tube and detector replacement The two components that end a system’s economic life, and the ones to ask about before you buy a used system
  • Room Lead shielding as the physicist’s plan requires, adequate space for the gantry sweep on a tomosynthesis system, and power to specification
  • Consumables and training Paddle covers, positioning aids, QC phantom, and continuing education for the technologists

None of this is a reason not to add mammography, and refurbished systems make the entry cost manageable. It is a reason to buy from a supplier that installs, calibrates and services the system and knows the MQSA paperwork, because a system that cannot pass its physics survey is not a bargain at any price. The room requirements are similar to a diagnostic X-ray room; our guide to X-ray room size and requirements covers the principles.

Key Comparison

2D vs 3D Mammography (Tomosynthesis) vs Breast CT

The biggest decision is 2D versus 3D, and there is now a third option for facilities that want cross-sectional breast imaging without compression.

TechnologyHow it worksWhere it fits
2D digital mammography (FFDM)Captures flat craniocaudal and mediolateral oblique images. Reliable and lower cost, still widely used for routine screeningCommunity screening programs, practices adding mammography for the first time, lower volume sites
3D mammography (tomosynthesis, DBT)Takes multiple low-dose images through an arc and reconstructs them into thin layers, giving radiologists a clearer view than 2D and reducing callbacks. The current standard of care in many facilities, at a higher priceFacilities serving dense-breast populations, diagnostic centers, and any site that wants tomosynthesis-guided biopsy
Breast CT (Koning Vera)A dedicated cone beam CT that images the entire breast from chest wall to nipple in a 10 second scan with no compressionDiagnostic workup, dense breasts and patients who cannot tolerate compression; a different class of system quoted on request

Tomosynthesis improves cancer detection especially in dense breast tissue, and it is the fastest-growing segment. The National Cancer Institute mammogram fact sheet covers the clinical evidence plainly. If you are building a program from scratch, buy the 3D-capable platform even if you license tomosynthesis later: a 2D-only system can rarely be upgraded to 3D, but a tomosynthesis-ready tube and gantry can be licensed when volume justifies it.

Used Market

Hologic, GE and Siemens Mammography Systems on the Used Market

Hologic, GE and Siemens dominate the installed base, so they dominate the used market, and a refurbished Selenia Dimensions, Senographe Essential or MAMMOMAT Inspiration is how many clinics add mammography affordably. We source and compare these systems on request. What to check before you buy any of them:

  • Detector Generation, calibration history and the most recent flat-field and phantom results. A detector nearing the end of its life turns a bargain into a replacement bill
  • Tube Exposure count and the date of the last tube change
  • Licences Tomosynthesis, synthesized 2D, contrast and biopsy options enabled on that serial number, and whether the manufacturer will transfer them
  • Software level Whether the platform is still supported, because an unsupported system can fail its next MQSA inspection on a software issue
  • Paddles and accessories The full paddle set, magnification stand and biopsy hardware, each of which is expensive to buy separately
  • Compliance records QC logs, physics surveys and the accreditation history of the room it came from

A refurbished system from Medical Outfitters is stripped, renewed where it needs renewing and tested against the manufacturer specification, then delivered, installed and calibrated with a written warranty. Our used imaging equipment inspection checklist covers the survey, and if you are replacing rather than adding we also buy used mammography systems.

Buying Decision

New vs Refurbished Mammography Machines

A new mammography system offers the latest detector, 3D software and a full manufacturer warranty. A certified refurbished mammography machine delivers the same image quality for far less, fully inspected and warrantied. Refurbished 3D systems in particular are a popular way for clinics to add tomosynthesis affordably. Refurbished typically saves 40 to 60 percent versus new.

The one place new wins outright is a program that needs contrast-enhanced mammography or the newest AI workflow, because those licences are tied to current platforms. For a screening program, a refurbished tomosynthesis system with a healthy detector and a full paddle set is usually the better economics; our page on refurbished medical imaging equipment explains what the words certified, refurbished and used mean.

Related Equipment

Breast Biopsy and Specimen Imaging

A complete breast-imaging program often pairs mammography with biopsy and specimen imaging. Medical Outfitters supplies breast biopsy systems such as the Mammotome line, and specimen imaging systems like the Kubtec Mozart for intraoperative margin assessment. Upright stereotactic and tomosynthesis-guided biopsy options on the mammography unit itself are quoted with the system.

Service & Support

Mammography Installation, MQSA Compliance and Service

Mammography systems require accreditation-compliant installation, regular QC phantom testing and MQSA-aligned service to stay certified. Medical Outfitters manages installation, calibration, service and parts, and the X-ray, fluoroscopy and mammography service page sets out the response commitments, what a preventive maintenance visit covers and how a contract is priced. Service coverage is not conditional on having bought the system from us.

  • Installation Room survey, rigging, placement, calibration to manufacturer specification and coordination of the physicist’s acceptance survey before the first patient
  • Preventive maintenance Detector calibration and flat-field checks, tube and generator output, compression force and thickness accuracy, mechanical checks on the gantry sweep and paddles, and the documentation your accreditation body and MQSA inspector ask for
  • Repair Same-day or next-day emergency response depending on urgency and location, with priority commitments for contract clients
  • Parts More than 20,000 parts in stock across MRI, CT, X-ray and more, shipped same day or next day
How to Choose

How to Choose the Right Mammography Machine

Match the system to your patient population, volume and whether 3D is required. Key takeaways:

  • 2D digital (FFDM) for reliable, lower-cost routine screening
  • 3D tomosynthesis for better detection, especially in dense breasts, and for tomosynthesis-guided biopsy
  • Buy the 3D-capable platform even if you license tomosynthesis later
  • Refurbished for 40 to 60 per cent savings with comparable image quality, on a platform the manufacturer still supports
  • Plan for biopsy, specimen imaging and reading monitors to complete the program
  • Budget the MQSA side: accreditation, the physicist and the QC program are part of the cost of owning the system

What is the difference between 2D and 3D mammography?
2D digital mammography captures flat images, while 3D tomosynthesis captures multiple thin layers for a clearer view, improving cancer detection especially in dense breast tissue.
What is 3D mammography (tomosynthesis)?
3D mammography, or tomosynthesis, takes multiple low-dose images from different angles and reconstructs them into thin layers, giving radiologists a clearer view than 2D and reducing callbacks.
Can a 2D mammography machine be upgraded to 3D?
Rarely on an older 2D-only platform, because tomosynthesis needs a tube and gantry that sweep through an arc and a detector fast enough to capture the projections. Tomosynthesis-ready platforms such as the Planmed Clarity 2D and the Fujifilm ASPIRE Cristalle can have the option licensed later, which is why we recommend buying the 3D-capable platform even if you start in 2D.
What does a mammography program cost beyond the machine?
MQSA certification and the annual inspection, accreditation by an FDA-approved body, a medical physicist’s acceptance survey and annual surveys, the technologist QC program, a reading workstation with 5 megapixel monitors, a service contract, and eventually tube and detector replacement. Room shielding follows the physicist’s plan.
Is a refurbished mammography machine worth it?
Yes. A certified refurbished mammography system saves 40 to 60 percent versus new with comparable image quality, provided it is fully inspected, backed by warranty and on a platform the manufacturer still supports. Check the detector generation, tube exposure count, enabled licences and compliance records before you buy.
What mammography brands do you carry?
Medical Outfitters sells the Fujifilm ASPIRE Cristalle and Planmed Clarity mammography systems new, with the Planmed Clarity 3D and the Fujifilm AMULET Innovality and Sophinity for markets outside the US, supplies the Koning Vera breast CT as an authorized Koning supplier, and sources refurbished Hologic, GE and Siemens systems on request.
Do you service mammography machines you did not sell?
Yes. Service coverage is not conditional on having bought the system from us. Our engineers install, calibrate and maintain mammography systems, coordinate the physicist’s surveys and keep the documentation your accreditation body and MQSA inspector ask for.
