How to choose a mammography machine
Five questions settle most mammography purchases before a model is discussed.
- Screening, diagnostic or both? A high-volume screening program lives on throughput and patient comfort. A diagnostic mammogram machine for a breast center needs tomosynthesis, biopsy guidance and, increasingly, contrast-enhanced mammography or breast CT for problem solving.
- 2D or 3D? Tomosynthesis has become the standard for new installations because it separates overlapping tissue, which matters most in dense breasts. A 2D system with an upgrade path is the budget route to the same place.
- Will you biopsy on the unit? Stereotactic and tomosynthesis-guided biopsy is an option on the Fujifilm AMULET Innovality and the Planmed Clarity systems, and the Koning Vera can biopsy on its own table.
- Which market? In the United States a mammography unit must be FDA approved or cleared for the exam type, and the facility certified under MQSA. That decides which systems in the grid below are open to you.

2D digital mammography or 3D tomosynthesis
2D full-field digital mammography (FFDM) captures a single projection of the compressed breast on a flat panel detector. It is fast, well understood and still the workhorse of many screening programs. 3D mammography, or digital breast tomosynthesis (DBT), sweeps the tube through an arc and reconstructs the breast as a stack of thin slices, so a lesion is no longer hidden behind the tissue above and below it. Detection improves most in dense breasts, and recall rates for overlapping tissue fall.
Two points decide the purchase. First, dose: a combined 2D plus 3D exam roughly doubles the dose of 2D alone, while a system that produces a synthesized 2D image from the tomosynthesis data brings a 3D exam back close to a 2D one. Every 3D system on this page offers synthesized 2D. Second, upgrade paths: if you plan to buy a tomosynthesis system eventually but not this year, the Planmed Clarity 2D and the Fujifilm Amulet Sophinity can be bought as 2D systems and upgraded to tomosynthesis later, which spreads the cost over two budgets.
A third option exists for diagnostic work. The Koning Vera breast CT images the breast as a true 3D volume with the patient prone and no compression at all, with a ten-second scan and biopsy on the same table. It is a diagnostic and problem-solving tool rather than a replacement for screening mammography.
New, refurbished or used mammography equipment
New brings the current detector generation, the latest tomosynthesis and contrast software, and the longest manufacturer warranty. Refurbished mammography machines from Medical Outfitters are inspected and tested, with the detector verified for image quality before delivery, and typically cost 40 to 60 percent less than new. The honest guidance: mammography detectors age, and a used unit is only as good as its detector and its service history. Room requirements and siting
A mammography room is one of the easier imaging rooms to build. The tube runs at low voltage, so the shielding a qualified medical physicist specifies is modest compared with a general X-ray room, and the footprint of a modern gantry fits a standard exam room. The Planmed Clarity S runs on single-phase power and folds for transport, which is why it suits mobile clinics.
What the room needs beyond the gantry: an acquisition workstation, a high-resolution reading workstation for the radiologist, PACS connectivity, and in the United States the MQSA framework, which means accreditation by an FDA-approved body, an annual survey by a medical physicist, and qualified technologists and interpreting physicians.
